<p>Governor Luis Fortuño law signed in December 191 as the "Act to prohibit the retention, storage and custody of certified copies of birth certificates." The Act provides that any certificate to be issued before 1 July this year, will be void from that date. However, the Vital Records issue a new format that seeks to prevent use in the street or stolen certificates are being used for identity theft.</p>
<p>To temper the law, said the official, the Population Registry will guide the public that no agency can seize the original copy of the certificate.</p>
<p>Act 191 provides that a person is sufficient to display the document. Only allow entities retain the photocopy or electronic or digital format of the document.</p>

1. Complete and sign the application. Send a photocopy of picture ID that is not expired. Only acceptable Drivers License and passport. These are subject to approval.<br />
<br />
2. Send money order for $ 5.00 or Internal Revenue stamp for each requested certificate payable to: Secretary of the Treasury of Puerto Rico. (Additional copies in the same order $ 4.00) No personal checks or other payments.<br />
<br />
3. Send pre-addressed stamped on and very clear direction to where it will receive the certificate. Our mailing address is: PO Vital Records Box 11,854 San Juan Puerto Rico 00910.<br />

The application can be made via prepaid express service (Express Mail, Federal Express, UPS) and back. <p>Many of our community members unfortunately will spend their holidays hospitalized and many will require blood, platelet or plasma transfusions. To meet their need we need donors to give to during the upcoming holiday season.</p>
<p>Please remember illnesses, accidents, diseases don’t take a holiday and neither can blood donations. Additionally the chapter is just short of goal for the month and we need more donors than normal to donate Friday, Saturday and Sunday. Making goal is important but saving a life, that is priceless.</p>
<p>In the spirit of thankfulness for your good health share your blessing with someone who needs it, by donating blood.&#160; If you cannot donate at this time, please share this with someone who can.</p>

         <content:encoded><![CDATA[<p><font face="Arial" size="3">RALEIGH — The North Carolina Division of Public Health has issued a fish consumption advisory for Badin Lake between Stanly and Montgomery counties. Elevated levels of PCBs have been found in large mouth bass and catfish in Badin Lake.&#160; </font>&#160;<br />
&#160;</p>
<p><font face="Arial" size="3">As statewide and regional mercury advisories for these fish have been in place since 2006, Public Health officials’ fish consumption advice will not change.&#160; Because of the PCB and mercury contamination, pregnant women, nursing women and children under age 15 should not eat <i>any</i> largemouth bass or catfish from the lake. Other people should not eat more than one meal a week of catfish or large mouth bass from Badin Lake. </font>&#160;<br />
<font face="Arial" size="3">PCBs are chemicals that used to be used as coolants and lubricants in transformers, capacitors and other electrical equipment. They were released into the environment during manufacturing and through improper storage or disposal of old electrical equipment. They are no longer made in the U.S. but persist in the environment. The fish testing in Badin Lake did not address the source of PCB contamination.</font>&#160;<br />
<font face="Arial" size="3">PCBs can cause anemia; acne-like skin conditions; damage to the liver, stomach or thyroid gland; changes in the immune system or reproductive system; and behavioral problems. In infants and children, PCBs may cause reduced birth weight, motor-skill problems, impaired short-term memory and impaired immune response.</font>&#160;<br />
<font face="Arial" size="3">The N.C. Division of Public Health analyzed Badin Lake environmental data and fish sampling data at the request of Stanly County officials. The fish were tested for PCBs and PAHs (polynuclear aromatic hydrocarbons), both of which can cause adverse health effects. No PAHs were detected in the fish, but concentrations of PCBs in some catfish and large mouth bass were greater than the public health level of concern, prompting the fish consumption advisory.</font>&#160;<br />

<p>&#160;<font face="Arial" size="3">RALEIGH - La División de Salud Pública de Carolina del Norte ha publicado una advertencia sobre el consumo de pescado del lago Badin que se ubica entre los condados de Stanly y Montgomery. Altos niveles de sustancias químicas conocidas como bifenilos policlorados o <i>PCB</i> en inglés (polichlorynated biphenils) fueron encontrados en el pez gato o bagre <i>(catfish)</i> y el pez huro o lobina boca grande <i>(large mouth fish)</i> del Lago Badin. &#160;<br />
&#160;<br />
Desde el 2006 se han emitido varias advertencias estatales y regionales sobre la contaminación con mercurio de estos peces. Funcionarios de salud pública señalan que las advertencias para el consumo de estos peces no van a cambiar y siguen en pie. Debido a la contaminación por mercurio y los PCB, las mujeres embarazadas, las mujeres que están amamantando y los niños menores de 15 años no deben comer pez gato/bagre o huro/lobina boca grande provenientes de este lago. Para la población en general se recomienda no consumir más de una vez a la semana pez gato/bagre o huro/lobina boca grande del Lago Badin. &#160;<br />
</font></p>
<p><font face="Arial" size="3">Los PCB son sustancias químicas que se utilizan como refrigerantes y lubricantes en transformadores, condensadores y otros equipos eléctricos. Estos fueron liberados en el medio ambiente durante el proceso de fabricación y por medio de almacenamiento inapropiado o eliminación de aparatos eléctricos viejos. Los PCB ya no se producen en los Estados Unidos, pero siguen en el medio ambiente. Las pruebas en los peces del lago Badin no proveen información sobre la posible fuente de contaminación de PCB. &#160;<br />
&#160;<br />
Los PCB pueden causar anemia, problemas en la piel parecidos al acné, daño al hígado, glándula tiroides o al estómago, así como cambios en el sistema inmunológico, sistema reproductivo y problemas de comportamiento. En los bebés lactantes y niños, los PCB pueden causar bajo peso al nacer, problemas de actividad motriz, problemas de memoria a corto plazo y disminución de la respuesta del sistema inmunológico. &#160;<br />
&#160;<br />
La División de Salud Pública de Carolina del Norte está analizando los datos ambientales y los peces de muestreo del Lago Badin por petición de los funcionarios del Condado de Stanly. Se les hicieron pruebas de PCB y de hidrocarburos aromáticos polinucleares (PAH en inglés) a los peces, ambas sustancias químicas pueden causar efectos adversos a la salud. No se detectaron PAH en los peces pero las concentraciones de PCB en los peces nombrados anteriormente fueron de alto nivel para la salud pública, provocando el pronunciamiento de esta advertencia de consumo de pescado. &#160;<br />
